The HECTD2:rs12249854 polymorphism may be associated with a weaker response to an intradetrusor injection of botulinum neurotoxin type A (BoNT/A) in patients with overactive bladder (OAB). This prospective study analyzed the link between the HECTD2:rs12249854 polymorphism and the response to BoNT/A treatment in 156 OAB patients, based on changes in urodynamic parameters and ICIQ-OAB and ICIQ-LUTS-QoL scores six months after the injection. BoNT/A treatment contributed to improvements in all parameters except the average flow rate. Wild-type homozygotes (TT, n = 137) differed from TA heterozygotes (n = 19) solely in terms of pretreatment ICIQ-OAB Part A and ICIQ-LUTS-QoL Part A scores, both of which were higher in the heterozygotes. Thus, the HECTD2:rs12249854 polymorphism does not appear to be associated with the response to BoNT/A treatment. Patients with neurogenic OAB (n = 60) had a higher pretreatment detrusor pressure at maximum flow than those with idiopathic OAB (n = 96), but this difference was no longer observed after BoNT/A treatment. Good responders to the BoNT/A injection (defined as at least 50% decrease in the ICIQ-LUTS-QoL Part A score, n = 50) had a higher body mass index and parity and a lower pretreatment maximum flow rate than poor responders (n = 106). Before formulating any definitive conclusions, the results of this exploratory study should be verified in an adequately powered multicenter cohort.
