PURPOSE OF REVIEW: To review recent advances in schema therapy for personality disorders, with particular emphasis on theoretical developments, clinical effectiveness, mechanisms of change, and implementation. RECENT FINDINGS: Recent trials further support schema therapy as an effective treatment for borderline personality disorder. Accumulating evidence also indicates broader applicability across cluster C and other personality disorders, and increasingly identifies clinically relevant changes in early maladaptive schemas and schema modes, particularly reductions in the Vulnerable Child mode and strengthening of the Healthy Adult mode, although their causal role remains uncertain. SUMMARY: Schema therapy is now supported by a substantial evidence base across borderline and several other personality disorders. Its integration of developmental theory with experiential interventions provides a distinctive therapeutic framework, although important questions remain regarding causal mechanisms, optimal delivery formats, and cross-cultural implementation.
