Why the study?
Early aldosterone synthase inhibitors lacked specificity over cortisol synthase and suppressed adrenal cortisol production, motivating evaluation of the effect of the selective inhibitor baxdrostat on adrenocorticotropic hormone-stimulated cortisol levels in uncontrolled hypertension.
Population
Patients with uncontrolled hypertension
Comparison
8-week exposure to baxdrostat
Design
Phase 2 study
Follow-up
8-week exposure
Phase 2 trial evaluates cortisol responsiveness after adrenocorticotropic hormone stimulation in uncontrolled hypertension, assessing baxdrostat selectivity over cortisol synthesis.
A ldosterone synthase inhibitors have demonstrated efficacy in lowering blood pressure in patients with difficult-to-control hypertension.1-3 Early aldosterone synthase inhibitors lacked specificity for aldosterone synthase (via the CYP11B2 gene) over its close homolog cortisol synthase (via the CYP11B1 gene), leading to suppressed adrenal cortisol production.4 Drug-induced cortisol insufficiency has not been detected in trials of highly selective aldosterone synthase inhibitors.1-5 We report the effect of 8-week exposure to baxdrostat on cortisol levels after stimulation with adrenocorticotropic hormone in patients with uncontrolled hypertension.
