The increasing number of elderly people demands promotive and preventive efforts to maintain physical health through participation in community-based programs. The Cangkol Elderly-Friendly Village Zakat House Program is a form of elderly empowerment that provides exercise activities, health education, and routine health checks. This study aims to determine the relationship between elderly participation and physical health as measured by physical activity and routine health checks in the Cangkol Elderly-Friendly Village Zakat House Program, Mojolaban District, Sukoharjo Regency. This study applies a quantitative analytical design with a cross-sectional approach. A total of 70 elderly people were involved as samples through a total sampling technique. Data collection was carried out using questionnaires and observation sheets, while data analysis used the Chi-Square test. The results showed that elderly participation was not related to physical activity (p = 0.193), but was related to routine health checks (p = 0.000). Active participation in the program plays a role in increasing the utilization of preventive health services, but has not been followed by an increase in daily physical activity. It was concluded that elderly participation is related to routine health checks, and not related to physical activity. Seniors are advised to practice independent physical activity and continue to undergo regular health check-ups. Program managers need to maintain health check-up services and strengthen health education to support healthy aging.
