Perioperative fluid balance after CABG is linked to pulmonary complications and ICU stay duration.
Why the study?
Postoperative pulmonary complications and prolonged ICU stay remain a concern after CABG despite improvements in surgical and perioperative care, and perioperative fluid balance is a modifiable factor that may significantly influence these outcomes.
Does perioperative fluid balance intra-operatively and during the first 48 hours post-operatively influence pulmonary complications and ICU length of stay in adult patients undergoing elective, isolated on-pump CABG?
Cohort (n=65)
No
Does perioperative fluid balance intra-operatively and during the first 48 hours post-operatively influence pulmonary complications and ICU length of stay in adult patients undergoing elective, isolated on-pump CABG?
Should not yet alter perioperative fluid management after CABG; leaves open whether balance causally drives complications.
Introduction: Coronary artery bypass grafting (CABG) remains a cornerstone in the management of advanced coronary artery disease.Despite improvements in surgical and perioperative care, postoperative morbidity-particularly pulmonary complications and prolonged ICU stay-continues to be a concern.Perioperative fluid balance is a modifiable factor that may significantly influence these outcomes.Method: This historical cohort study included adult patients undergoing elective, isolated on-pump CABG at a tertiary cardiac center.Preoperative demographics, intraoperative variables, and postoperative fluid balance were collected.Fluid balance was calculated intra-operatively and during the first two postoperative days.Pulmonary complications and ICU length of stay were recorded.Statistical analysis was performed using appropriate univariate and multivariable methods, with significance set at p < 0.05.

Shah et al. (2026) conducted a cohort in Coronary artery disease undergoing elective on-pump coronary artery bypass grafting (CABG) (n=65). Perioperative fluid balance (intra-operative and 48-hour post-operative) vs. Lower fluid balance / across fluid balance categories (<2000 mL, 2000-3000 mL, >3000 mL) was evaluated on Length of ICU stay (correlation with intra-operative fluid balance) (r = -0.287, p=0.020). Perioperative fluid balance after CABG was significantly associated with pulmonary complications (atelectasis 16.9%) and ICU stay duration (intra-op r=-0.287, p=0.020; day 2 r=-0.314, p=0.011).
