Perioperative hemodynamic and anesthetic management modulates myocardial excitability and ventricular arrhythmogenesis.
This review discusses how perioperative myocardial excitability and ventricular arrhythmogenesis can be modulated through hemodynamic management, anesthetic agents, and regional anesthetic or surgical techniques.
Cardiac arrhythmias are associated with significant morbidity and mortality. Ventricular arrhythmogenesis is a dynamic interplay among an arrhythmogenic substrate, myocardial electrophysiologic properties, modifying factors, and triggering factors. Imbalances in the autonomic nervous system can lead to increased myocardial excitability, which is a major contributor to the pathophysiology of ventricular tachyarrhythmias. Myocardial excitability and ventricular arrhythmogenesis are modulated perioperatively through hemodynamic and physiologic management, anesthetic agents, or regional anesthetic and surgical techniques.

Howard-Quijano et al. (2026) conducted a review in Perioperative ventricular arrhythmias and myocardial excitability. Perioperative hemodynamic/physiologic management, anesthetic agents, and regional anesthetic/surgical techniques was evaluated on Ventricular arrhythmogenesis and myocardial excitability. Perioperative hemodynamic management, anesthetic agents, and regional anesthetic or surgical techniques modulate myocardial excitability and ventricular arrhythmogenesis.
