A 5-year-old boy with high-risk neuroblastoma underwent 68Ga-DOTATATE PET/CT as part of a clinical trial to assess treatment response after anti-GD2 immunotherapy. The scan showed symmetric increased uptake in bilateral femora and tibiae with periostosis. 18F-MFBG PET/CT revealed no abnormal uptake, and no metastasis was found. The diagnosis was hypertrophic osteoarthropathy. This case demonstrates that hypertrophic osteoarthropathy can occur as a rare complication after anti-GD2 therapy and can be detected by 68Ga-DOTATATE PET/CT.
