Aim: To evaluate perioperative and oncological outcomes comparing the thoracoabdominal (TAA) approach and total abdominal (TA) approach for resection of oesophagogastric junction adenocarcinoma (CA OGJ) over a 5-year period at a single center. Methods: A retrospective review was conducted on patients undergoing curative-intent surgical resection for CA OGJ between June 2021 and June 2026.Patients were stratified by surgical approach (TAA vs. TA).Operative parameters, postoperative morbidity, histological margin status, and overall survival were analyzed. Results: A total of 26 patients were identified: 5 (19.2%) underwent TAA and 21 (80.8%)underwent TA approach.Baseline age was similar between groups (TAA: 71.6 vs. TA: 70.0 years).R0 resection rate was 60.0% (3/5) in the TAA group compared to 85.7% (18/21) in the TA group.Mean OT duration was longer in the TAA group (387.2 vs. 320.1 min), with higher intraoperative blood loss (424.0 vs. 189.5 mL).Postoperative complications occurred in 60.0% (3/5) of TAA patients versus 38.1% (8/21) of TA patients, with pulmonary complications present in 40.0% vs. 19.0%,respectively.Median postoperative length of stay was 18.0 days for TA vs. 16.0 days for TAA.Overall 30-day mortality was 0% across both cohorts. Conclusion: In our 5-year institutional experience, the total abdominal approach was predominantly utilized for CA OGJ resections and demonstrated shorter operative time, lower blood loss, and comparable resection margins relative to the thoracoabdominal approach. These findings support the safety and feasibility of the total abdominal approach in selected CA OGJ cases.
