We read with interest the article by Arredondo et al.1 reporting the final results of the ELECLA trial, a randomized controlled trial comparing neoadjuvant chemotherapy (NAC) followed by surgery with upfront surgery in 127 randomly assigned (120 analyzed in the modified intention to treat population) patients with locally advanced colon cancer (LACC), defined as any T stage with suspected nodal disease or cT4, or cT3 with >5 mm extramural invasion. The authors concluded that NAC was safe, feasible, and associated with improved downstaging without increased perioperative morbidity.
