Background “O-pitched” spring-type threads have tightly wound coils that lie in direct contact and require no rotation during insertion, potentially helping preserve thread shape and placement. This study evaluated the clinical outcomes of midface rejuvenation with these threads over a follow-up period of up to 24 months. Methods We retrospectively reviewed seven patients who underwent midface rejuvenation with “O-pitched” spring-type threads and had a mean follow-up of 16.3 months (range, 10–24 months). Two independent evaluators graded all available photographs using the Global Aesthetic Improvement Scale (GAIS) and Medicis Midface Volume Scale (MMVS). Four patients had photographs suitable for paired baseline and 12-month comparisons. Satisfaction was assessed in all patients at approximately 12 months; for the two patients without a 12-month visit, the last available scores, recorded at 10 and 11 months, were used. Results All seven patients were female. The median MMVS grade improved from 3 at baseline to 2 at 12 months. Both evaluators recorded a one-grade improvement in three of the four patients; one evaluator recorded the same improvement in the fourth. At 12 months, the median GAIS score was 4 (“much improved”), and mean satisfaction was 4.1 out of 5. No major complication was documented. The only recorded minor effects were transient pain and bruising, which occasionally lasted up to 1 month. Some improvement remained visible at 24 months in one illustrative case. Conclusions “O-pitched” spring-type threads may improve midface volume and contour in selected patients. Favorable changes were observed at 12 months, but their magnitude and durability varied among patients.
