Background and Objective: The purpose of this study was to evaluate the efficacy of high-dose aflibercept 8 mg in the treatment of diabetic macular edema (DME) refractory to faricimab-svoa treatment. Patients and Methods: This retrospective study included 82 eyes from 64 patients with DME previously treated with faricimab-svoa. Injection intervals, corrected or pinhole visual acuity (VA), and central foveal thickness (CFT) were recorded. Results: Between the time of switch to the fifth injection, the average (SD) injection interval increased from 63 (45) days to 69 (23) days ( P = .01). Mean VA decreased from 58.1 (23.1) to 52.9 (22.7) letters ( P = .263), and average CFT decreased from 360 (145) µm to 348 (151) µm ( P = .626). Intraretinal fluid incidence decreased from 50% to 33.3% ( P = .066), while subretinal fluid incidence increased from 6.1% to 14.8% ( P = .308). Conclusion: Anatomic and visual changes were not significant following switch to aflibercept 8 mg. Sustained injection interval improvements were present at the final aflibercept 8 mg injection.
