Cutaneous cytomegalovirus (CMV) infection is a rare entity, despite a significant seroprevalence of CMV globally. It is most often encountered as an opportunistic infection among immunosuppressed individuals. Skin involvement in CMV infection could be the heralding sign of a more disseminated disease. We report a case series of five immunocompromised patients who manifested varied morphological features of cutaneous CMV infection. All patients were diagnosed based on histopathology, supplemented with immunohistochemistry or serology. The clinical presentations seen in our patients include punched-out ulcers in the perineal region and distal extremities, vesicobullous lesions, truncal ulcers with scalloped margins, crusted hyperpigmented papules, and oral erosions. After a thorough evaluation, patients were initiated on oral valganciclovir or intravenous ganciclovir. Being a rarity, this case series highlights the clinical manifestations and management of cutaneous CMV infection.
