Fecaloma is a rare cause of large bowel obstruction in pediatric and usually occurs in the rectosigmoid colon. Plain abdominal radiography is important for early diagnosis. A 10-year-old male was admitted with abdominal pain and progressive abdominal distension. Plain abdominal radiograph showed dilated colon and a large fecal mass in the rectosigmoid region with a mottled gas appearance, suggesting fecaloma causing large bowel obstruction. Plain abdominal radiography can reliably diagnose fecaloma by showing its characteristic appearance. Early recognition is important to allow timely management and prevent complications.
