Missed specialist referrals in primary care pose substantial health and medico-legal risks, yet remain poorly understood. We developed an AI-supported safety system (MR-AI) to identify missed referrals from consultation notes, evaluated in a quasi-experimental study across seven primary care clinics in Singapore. MR-AI scanned ~25,000 notes weekly for referral intent without a corresponding order, achieving an estimated sevenfold increase in weekly missed referral detection (4.7 percentage point level increase; 95% CI 0.024–0.069; p < 0.001). Of 428 cases flagged, 225 were clinician-verified, alongside 31 patient-reported cases, yielding 256 total detections. MR-AI demonstrated 99.9% specificity and 48.8% sensitivity. In a blinded one-week comparison against seven physician reviewers, MR-AI achieved a PPV of 61.8% versus a human confirmation rate of 51.8% across 83 flagged cases, while requiring approximately 41 fewer projected staff-hours. AI-enabled surveillance substantially assisted in missed referral detection, though sustainable improvement will require complementary preventive workflow changes and future cost-effectiveness assessment.
