Background: Sheetapitta is a Tridoshaja Vyadhi predominantly involving Vata and Kapha with Pitta. It closely resembles Urticaria, a hypersensitivity disorder characterized by recurrent wheals, pruritus, and erythema, affecting quality of life. Conventional treatment mainly provides symptomatic relief with recurrence. Ayurveda recommends Shodhana and Shamana therapies. Hence, this study evaluated the combined effect of Sadhyovamana, Gudadeepyaka, and Nishottamadi Kashaya in Sheetapitta vis-à-vis Urticaria. Objectives: To evaluate the Combined efficacy of Sadhyovamana followed by Gudadeepyaka and Nishottamadi Kashaya in reducing clinical manifestations and improving the Urticaria Activity Score (UAS). Materials and Methods: An open-label single-arm clinical study was conducted on 33 subjects diagnosed with Sheetapitta vis-à-vis Urticaria, with 31 completing the trial. Patients underwent Sadhyovamana followed by Gudadeepyaka and Nishottamadi Kashaya. Assessments were performed on Days 0, 3, 31, and 45. Outcomes were evaluated using UAS and symptom scores for Varati-Damshtravat Shotha, Kandu, Toda, Daha, and associated symptoms. Statistical analysis was performed using nonparametric tests. Results: The intervention produced highly significant reductions in UAS and symptom scores (p < 0.001). Marked improvement from Day 3 was sustained throughout follow-up. Relief was noted in wheals, pruritus, burning, and pricking pain. Most participants experienced improvement without serious adverse events. Conclusion: The combined administration of Sadhyovamana, Gudadeepyaka, and Nishottamadi Kashaya was safe and effective in managing Sheetapitta vis-à-vis Urticaria. The treatment provided rapid symptomatic relief, reduced disease activity, and maintained clinical improvement during follow-up, supporting its potential as an Ayurvedic therapeutic approach in this clinical study.
