Gingival recession presents significant esthetic, functional, and structural challenges in periodontology. While subepithelial connective tissue grafts (SCTGs) remain the gold standard for root coverage, their application is limited by donor site morbidity and tissue availability. This systematic review comprehensively evaluates the biological profile and clinical efficacy of injectable platelet-rich fibrin (i-PRF) as a primary or adjunctive therapy for gingival recession coverage. By analyzing the synergistic effects of i-PRF with connective tissue grafts, collagen membranes, free gingival grafts, and standalone semisurgical methodologies, this review demonstrates that i-PRF significantly enhances keratinized tissue width and thickness, accelerates neo-angiogenesis, and provides predictable root coverage while minimizing postoperative morbidity.
