Association between Health Literacy and Blood Pressure Control and Factors Associated with Blood Pressure Control among Adults Receiving Antihypertensive Medication: Analysis of Data from the 2023–2024 Korea National Health and Nutrition Examination Survey

초록·키워드 본 연구는 약물 치료 중인 성인 고혈압 환자의 건강정보이해능력과 혈압 조절 간의 관련성 및 혈압 조절 관련 요인을 파악하기 위한 횡단적 이차자료 분석 연구이다. 제9기 국민건강영양조사(2023, 2024년)에 참여한 고혈압 약물 치료 중인 19세 이상 성인 2,587명을 대상으로 복합표본 빈도분석, 기술통계, 카이제곱 검정, 일반선형모형, 로지스틱 회귀분석을 시행하였다. 건강정보이해능력은 표준화된 한국형 측정도구로 평가하였고, 수축기 혈압이 140mmHg 미만이면서 이완기 혈압이 90mmHg 미만인 경우를 혈압 조절군으로 정의하였다. 연구 결과 혈압 조절률은 79.6%였으며, 건강정보이해능력 점수는 혈압 조절군이 미조절군보다 유의하게 높았다. 미보정 복합표본 로지스틱 회귀분석 결과 건강정보이해능력 점수가 1점 높을 때 혈압 조절 승산은 1.03배높았으나(OR=1.03, 95% CI=1.01-1.05, p=.015), 관련 특성을 보정한 Model 2와 Model 3에서는 유의한 관련성이 확인되지 않았다. 최종 보정 모형(Model 3)에서 연령만이 유의한 관련 요인이었으며, 65세 이상군에 비해 50세 미만군(OR=2.59, 95% CI=1.49~4.51, p=.001)과 50~64세군(OR=1.49, 95% CI=1.15~1.91, p=.002)에서 혈압 조절 승산이 유의하게 높았다. 건강정보이해능력은 보정 후 혈압 조절과 유의한 관련성이 없었으며, 연령만이 유의한 관련 요인으로 확인되었다. 따라서 특히 고령 환자의 혈압 조절 상태를 면밀히 평가하고 연령 특성을 고려한 관리 전략을 마련할 필요가 있다. This study examined the association between health literacy and blood pressure (BP) control and factors associated with BP control among adults receiving antihypertensive medication. Data from 2,587 adults aged ≥19 years in the Ninth KNHANES (2023–2024) were analyzed using complex-sample descriptive statistics, chi-square tests, general linear models, and logistic regression. Controlled BP was defined as systolic BP <140 mmHg and diastolic BP <90 mmHg. The BP control rate was 79.6%, and participants with controlled BP had higher health literacy scores. Health literacy was associated with BP control in the unadjusted model (OR=1.03, 95% CI=1.01-1.05, p=.015), but not after adjustment. In the final model, age was the only significant factor. Compared with adults aged ≥65 years, those aged<50 years (OR=2.59, 95% CI=1.49-4.51, p=.001) and 50–64 years (OR=1.49, 95% CI=1.15-1.91, p=.002) had higher odds of BP control. Health literacy was not independently associated with BP control after adjustment, whereas age was.
