Daily self-care, encompassing dietary and exercise practices, holds important significance in preventing and rehabilitating cardiovascular diseases (CVDs). However, a big challenge in daily self-care lies in how to acquire reliable self-care knowledge and effectively translate it into practical healthcare implementation. To bridge this gap, this paper introduces Petri net for ontology-based knowledge representation. By leveraging individual health status as input, we construct a fuzzy Petri net-based reasoning network to generate personalized self-care programs for diet and exercise. Furthermore, taking into account user preferences and daily routines, an optimization model is employed to quantify specific dietary choices and exercise durations. A case study is then presented to further illustrate the application of our proposed approach in generating tailored self-care programs. This study exhibits the potential to assist individuals in accessing more relevant and actionable diet and exercise guidelines.
