This paper presents a description of a surgical technique—a modified Henning‐Piontek procedure—used for meniscal root salvage. The method is intended for difficult, chronic injuries with loss of continuity of the meniscal root. Such cases would most often result in total or subtotal meniscectomy, which, as is well known, significantly accelerates the degenerative changes in the knee. This technique aligns with the “save the meniscus” approach practiced and promoted by the authors. It involves the use of an iliotibial band graft to create a bridge between the damaged meniscus and the anatomical attachment site of the meniscal root. This enables anatomical repair of the meniscal root, facilitates healing, and preserves the natural structure of the joint.
