The National Health Insurance (JKN) program administered by the Social Security Agency for Health (BPJS Kesehatan) aims to provide equitable healthcare access for the entire population. However, high enrollment rates have not been fully matched by optimal utilization of primary healthcare facilities. This study aims to analyze the factors associated with healthcare service utilization among BPJS participants at the Lapandewa Public Health Center, South Buton Regency. The study employed an observational quantitative analytical method with a cross-sectional approach. The study population encompassed all registered BPJS participants within the Lapandewa Public Health Center working area, yielding a sample size of 97 respondents calculated using the Slovin formula and selected via purposive sampling. Data were analyzed using univariate, bivariate (chi-square test), and multivariate (multiple logistic regression) procedures. The bivariate analysis revealed that participant attitude (p = 0.000) and family support (p = 0.000) were significantly associated with healthcare utilization, whereas household income (p = 0.321) and BPJS membership category (p = 0.500) showed no significant association. The multivariate model confirmed that attitude (p = 0.001; Exp(B) = 119.318) and family support (p = 0.031; Exp(B) = 15.909) were independent dominant predictors of healthcare utilization. In conclusion, fostering positive attitudes and strengthening family involvement are essential to optimize primary healthcare utilization.
