Multi-agent immunosuppression improves LVEF to ~40-45% in steroid-refractory ICI myocarditis before fatal myasthenia.
Why the study?
Immune checkpoint inhibitor-associated myocarditis is a serious immune-related adverse event with a variable clinical presentation and challenging management.
Case Report (n=1)

This case demonstrates that while combination immunosuppression with high-dose corticosteroids, mycophenolate mofetil, and infliximab can achieve cardiovascular stabilization in steroid-refractory ICI myocarditis, concurrent neuromuscular toxicity can still progress to a fatal myasthenic crisis.

May support triple immunosuppression for LVEF recovery in refractory ICI myocarditis; leaves open optimal regimen and prognosis.
Immune checkpoint inhibitor (ICI)-associated myocarditis is a serious immune-related adverse event with a variable clinical presentation and challenging management. We present a case of fulminant ICI-associated myocarditis complicated by complete heart block, decompensated heart failure, and cardiogenic shock, followed by neuromuscular toxicity with myasthenia gravis overlap. Our case highlights the clinical challenges of ICI myocarditis and a possible treatment strategy involving high-dose steroids and T-cell antimetabolites.

Dillon et al. (2026) conducted a case report in Immune checkpoint inhibitor-associated myocarditis and myasthenia gravis (n=1). High-dose corticosteroids, mycophenolate mofetil, and infliximab was evaluated on Clinical and cardiovascular stabilization (LVEF and high-sensitivity troponin I) and survival.
