Cardiac rehabilitation exercise prescription carries substantial uncertainty across 4 clinical domains, notably HRmax estimation.
Why the study?
The pathways through which uncertainties such as measurement errors and physiological variability enter cardiac rehabilitation exercise prescriptions required examination to establish a framework for clinical interpretation and minimum reporting.
This methodological review demonstrates that fixed target heart rate zones in cardiac rehabilitation are subject to cumulative measurement, estimation, physiological, and transfer uncertainties, proposing an Uncertainty Chain framework and hierarchical decision ranges to improve prescription safety and clinical interpretation.

Clinicians should interpret heart rate-guided prescriptions cautiously in cardiac rehabilitation; leaves open validation of adjusted frameworks in prospective trials.
OBJECTIVES This study examines the pathways through which uncertainties—such as measurement errors and physiological variability—enter cardiac rehabilitation exercise prescriptions, proposing a framework for clinical interpretation and minimum reporting.METHODS We searched PubMed/MEDLINE (2000-2026) and synthesized 39 core sources regarding prescription errors and variability, conducting a structured methodological narrative review guided by SANRA transparency standards.RESULTS Prescription uncertainty was classified into four domains: measurement, estimation, physiological variability, and application/transfer uncertainty, which ultimately converge on clinical decision-making. Specifically, predictive equations for maximum heart rate in patients on beta-blockers, ventilatory threshold interpretations in heart failure, and wrist-worn device measurements exhibited substantial clinical deviations.CONCLUSIONS Fixed target zones must be interpreted as evidence-qualified decision ranges specifying input provenance and potential errors, rather than absolute physiological boundaries. The proposed Uncertainty Chain model contributes to enhancing prescription safety, though future prospective clinical validations are required.

Wanyoung Yoon (2026) conducted a review in Cardiovascular disease requiring cardiac rehabilitation. Exercise intensity prescription in cardiac rehabilitation (using HRmax, HRR, VO2R, VT, RPE, and wearable monitoring) vs. Direct cardiopulmonary exercise testing (CPET) and electrocardiography (ECG) reference standards was evaluated on Classification and synthesis of exercise prescription uncertainty domains (measurement, estimation, physiological variability, and application/transfer). Exercise intensity prescription in cardiac rehabilitation involves 4 uncertainty domains—including beta-blocker HRmax prediction (SEE 18 bpm) and wrist PPG limits (-21.05 to 18.56 bpm).
