Pancreatic ductal adenocarcinoma (PDAC) is a highly lethal malignancy, frequently diagnosed at an advanced stage. While systemic chemotherapy is the standard of care for metastatic disease, clinical outcomes vary considerably among patients. Oligo-recurrence, limited metachronous metastasis potentially suitable for aggressive local treatment, has emerged as a clinically relevant concept associated with prolonged survival in selected patients, while the prognostic significance of synchronous oligometastasis, limited metastatic disease present at initial diagnosis, remains poorly characterized in PDAC.
