This research was motivated by the high risk of cargo hold flooding on bulk carrier vessels, which can threaten operational safety, ship stability, and crew safety. The cargo hold flooding incident on MV. Prima Sentosa indicated problems involving the ballast system, hatch cover, bilge pipes, and the implementation of ship safety procedures. This condition requires further examination because water entering the cargo hold may develop into progressive flooding and reduce ship stability due to the free surface effect. This study aims to identify the factors causing cargo hold flooding, analyze its impacts on ship operational safety, and evaluate prevention and handling measures based on operational procedures and the ship safety management system. A qualitative approach with a case study design was employed through observations, interviews with the Captain, Chief Officer, and Bosun, as well as documentation studies. The data were analyzed descriptively to identify the technical, operational, and managerial factors contributing to cargo hold flooding.
