The paper presents a case report of patient Yu., 47 years old, with multiple shrapnel perineal lesions and extraperitoneal injury of the rectum, extensive phlegmon of the perirectal tissue and pelvic floor muscles due to combat trauma after surgical treatment of phlegmon in combination with the injection autologous transplantation of adipose tissue components. This clinical case clearly demonstrates the possibility of autologous adipose tissue use in the treatment of perineal wound defects. The components of the stromal vascular fraction contained in adipose tissue contribute to the reduction of the rehabilitation period of patients by accelerating the epithelialization of wound defects.
