When the Neurology® Podcast debuted in September 2007, podcasting occupied the fringes of academic medicine. Smartphones had only just emerged, and streaming audio was uncommon. Nearly 2 decades later, the Neurology Podcast stands among the longest-running physician-hosted podcasts in medicine and has fundamentally expanded how one of the world's leading neurology journals educates its readers. Although the technologies through which neurologists consume information have changed dramatically since 2007, the podcast's enduring success reflects a remarkably constant mission: to deliver practical, clinically relevant education through authentic conversation. This article traces the origins and evolution of the Neurology Podcast through archival documents, contemporaneous publications, and personal recollections from those who created it.
