Background: A culturally adapted and validated Transfusion-Dependent Quality of Life (TranQol) tool is essential for evaluating health-related quality of life in children with Transfusion-Dependent Thalassemia (TDT). This study aimed to validate the Bangla version of the Children’s TranQol questionnaire by assessing its factor structure, internal consistency and convergent validity in Bangladeshi children with TDT. Materials and methods: A cross-sectional study was conducted among 50 children aged 2–18 years at the transfusion centers of Chittagong Medical College Hospital and Chattagram Maa-O-Shishu Hospital from January 2025 to June 2025. The questionnaire underwent a three-phase adaptation process: forward translation, back-translation, and expert review with Focus Group Discussions, followed by piloting and data collection. Psychometric evaluation included Exploratory Factor Analysis, Cronbach’s alpha and Pearson’s correlations for convergent validity, KMO and Bartlett’s tests to confirm data suitability for factor analysis. Results: The 28-item scale demonstrated excellent reliability with a Cronbach’s alpha of 0.936. Item–total correlations ranged from 0.053 to 0.856, with four items (Q1, Q7, Q15, Q20) showing weaker correlations (<0.30) though removing them did not improve reliability. EFA identified four factors with eigenvalues >1, explaining 58.6% of total variance, supporting construct validity. Sampling adequacy was confirmed (KMO = 0.722) and Bartlett’s test was significant (χ²(378) = 1233.569, p < .001). Pearson’s correlations were generally strong, indicating good convergent validity. Conclusion: The Bangla version of the Children’s TranQol questionnaire is a valid and reliable instrument for assessing quality of life in children with TDT and is suitable for both clinical use and research settings in Bangladesh.
