Segmental atrophy of the liver (SAL) is a benign pseudotumor with distinctive pathological features. This report details the clinicopathological findings in a patient confirmed by surgical pathology. Histological examination revealed thick-walled vascular proliferation, reactive bile duct hyperplasia, and stromal elastosis. Immunohistochemistry (Ki-67 index ∼1%, p53 negative) further supported a benign diagnosis. The patient underwent surgical resection for a left hepatic lobe mass and recovered well postoperatively. This report underscores that the integrated application of histomorphology and immunohistochemistry is crucial for the accurate diagnosis of such lesions, thereby preventing clinical misdiagnosis as malignancy.
