Aneurysmectomy with primary end-to-end anastomosis resolved all sensory symptoms in a 78-year-old woman with a 24-mm true extracranial internal carotid artery aneurysm.
Why the study?
Extracranial internal carotid artery aneurysms are rare, and their management relies on retrospective series rather than dedicated guidelines.

Carotid dolichoarteriopathy can enable primary end-to-end native-vessel reconstruction without graft interposition for extracranial internal carotid artery aneurysms, and atypical cervical sensory neuropathies should prompt vascular imaging.
Background Extracranial internal carotid artery aneurysms (EICAAs) are rare, and management relies on retrospective series rather than dedicated guidelines. Case A 78-year-old woman presented with cervical pain and left periauricular and mandibular-angle hypoesthesia with paresthesias from great auricular nerve (GAN) compression, an atypical sensory presentation, caused by a 24-mm true saccular EICAA (Attigah type I) with bilateral carotid dolichoarteriopathy. Aneurysmectomy with primary end-to-end anastomosis was performed without graft interposition, enabled by arterial redundancy; histopathology confirmed a true aneurysm, and symptoms resolved completely. Conclusion Carotid dolichoarteriopathy can enable durable native-vessel reconstruction, and unusual cervical sensory neuropathies should prompt vascular imaging.

Canto et al. (2026) conducted a case report in Symptomatic true saccular extracranial internal carotid artery aneurysm (Attigah type I) with great auricular nerve compression and bilateral carotid dolichoarteriopathy (n=1). Aneurysmectomy with primary end-to-end anastomosis without graft interposition was evaluated on Symptom resolution. Aneurysmectomy with primary end-to-end anastomosis resolved all sensory symptoms in a 78-year-old woman with a 24-mm true extracranial internal carotid artery aneurysm.
