Multimodality CT and radiography identify scimitar syndrome with preserved RV function in recurrent wheeze.
Why the study?
Scimitar syndrome has infantile and childhood/adult variants, and this report describes its presentation and imaging findings in an adolescent with recurrent wheeze.
Multimodality imaging (chest radiography and contrast-enhanced CT) can incidentally identify the childhood/adult variant of scimitar syndrome with characteristic anomalous pulmonary venous drainage and right lung hypoplasia in an adolescent presenting with acute asthma exacerbation, without right ventricular dysfunction or pulmonary hypertension.

Normal RV function and absent PH do not exclude scimitar syndrome in adolescent wheeze; this case extends the variant spectrum but leaves open further characterization.
Scimitar syndrome has infantile and childhood/adult variants. We describe an adolescent girl with recurrent wheeze in whom chest radiography and multimodality CT demonstrated the characteristic scimitar vein and associated bronchovascular abnormalities, with normal right ventricular function and no pulmonary hypertension.

Ing et al. (2026) studied this question. Chest radiography and multimodality CT identified a characteristic scimitar vein and bronchovascular abnormalities with preserved right ventricular function in an adolescent with recurrent wheeze.
