BACKGROUND Perioperative glycemic instability is common in patients with type 2 diabetes mellitus undergoing laparoscopic cholecystectomy.Continuous glucose monitoring (CGM) provides detailed glucose profiles, but evidence regarding CGM-guided perioperative management and the clinical meaning of perioperative time in range (TIR) remains limited.AIM To compare perioperative clinical outcomes of CGM-guided vs fingertip glucose-guided management and to explore the association between perioperative TIR and short-term postoperative events.METHODS This single-center retrospective cohort study included 240 patients treated between May 2023 and July 2025 (120 CGM-guided and 120 fingertip glucose-guided before matching).
