Macropolycytes are tetraploid polymorphonuclear granulocytes with various significance hypothesized. A retrospective review from a single centre of all the documented cases of macropolycytes in the peripheral blood from 2017 to 2024 was carried out for descriptive statistics. There were 30 cases. The aetiologies included infections (n = 7), rheumatoid disorders (n = 6), post-haematopoietic cell transplantation (n = 5), post-chemotherapy (n = 3), post-corticosteroids (n = 3), acute haemolysis (n = 3), and others (n = 3). All patients had either neutrophil predominance on differential count (n = 29), granulocytic left-shift (n = 28), or leucoerythroblastic reaction (n = 9). The occurrence of macropolycytes parallels granulocytic proliferation and per se may not be of any significance.
