FIB-4 triage and clinical phenotyping guide incretin-based therapy and SGLT2 inhibitor selection in MASLD.
Why the study?
Metabolic dysfunction-associated steatotic liver disease often coexists with obesity, diabetes, cardiovascular disease, and CKD, yet cardiovascular clinics frequently do not assess liver fibrosis risk, and combination incretin and SGLT2 inhibitor therapy lacks prospective validation.

This narrative mini-review proposes a clinical decision framework for patients with MASLD using FIB-4 risk triage and phenotype stratification (obesity/metabolic burden, ASCVD, or HF/CKD) to guide the prioritization of incretin-based therapies and SGLT2 inhibitors.
Metabolic dysfunction-associated steatotic liver disease often coexists with obesity, diabetes, cardiovascular disease, and chronic kidney disease (CKD).However, cardiovascular clinics often do not assess liver fibrosis risk.Incretin-based therapies can reduce cardiovascular events and improve steatohepatitis histology.Sodium-glucose cotransporter 2 inhibitors have hard endpoint benefits in heart failure (HF) and CKD.Combination therapy has only observational signals and lacks prospective validation. This review proposes a fibrosis-4 index based triage to identify three phenotypes: Obesity/metabolic burden, cardiovascular disease, and HF/CKD.Fibrosis stage is used as an independent modifier.

Qi et al. (2026) conducted a review in Metabolic dysfunction-associated steatotic liver disease (MASLD) with cardiovascular and metabolic comorbidities. Incretin-based therapies (GLP-1 receptor agonists, GIP/GLP-1 dual and triple agonists) and sodium-glucose cotransporter 2 inhibitors (SGLT2i) was evaluated. Incretin-based therapies and SGLT2 inhibitors can be prioritized in MASLD using FIB-4 risk triage and clinical phenotyping across obesity, ASCVD, and heart failure/CKD.
