Does intracoronary hematopoietic cell injection reduce left ventricular volumes during long-term follow-up in patients with old myocardial infarctions?
Intracoronary hematopoietic precursor cell injection in patients with old myocardial infarction improved left ventricular remodeling by reducing ESV and EDV and increasing infarct zone systolic wall thickening up to 5 years.

May support cell therapy for chronic MI remodeling; leaves open larger trials before practice change.
We investigated whether intracoronary hematopoietic cell injection reduces ventricular volumes during long-term follow-up in patients with old myocardial infarctions. Hematopoietic precursor cells were isolated via blood apheresis, and the concentrations of CD34+ and CD133+ cells were quantified. Through a 1:1 randomization, 16 patients received the apheresis product in the infarct-related coronary artery, while 16 patients served as the control group. Cardiac magnetic resonance imaging (MRI) was used to assess left ventricular (LV) volumes. In the cell treatment group, we observed a reduction in both end-systolic volume (ESV) and end diastolic volume (EDV) at the 1-year follow-up, which stabilized at the 5-year follow-up. This reduction was accompanied by an significant increase in systolic wall thickening (SWT) within the infarct zone. Conversely, the control group exhibited no changes in EDV, ESV, or ejection fraction (EF) at the 5-year follow-up.

Merino et al. (2026) conducted an RCT in Old myocardial infarction / old myocardial scars (n=32). Intracoronary hematopoietic precursor cell injection (CD34+ and CD133+ cells) vs. Control group (no cell injection) was evaluated on Left ventricular end-systolic volume (ESV) and end-diastolic volume (EDV) assessed by cardiac MRI. Intracoronary hematopoietic cell injection reduced left ventricular ESV and EDV at 1 year with stabilization at 5-year follow-up compared with no change in controls (n=32).
