The article presents a surgical experience of treating 38 patients with parastomal hernias following abdominal-perineal extirpation of the rectum. Depending on the severity of abdominal wall destruction, intraperitoneal underlay repair with composite meshes or a modified Pauli technique was used. Special attention was given to preventing mesh–bowel contact by covering the prosthesis with a peritoneal flap. Postoperative complications occurred in 15.79 % of patients, with no mesh removal required. During a follow-up period of up to 10 years, no hernia recurrences were observed. Individualized selection of repair techniques and meticulous mesh fixation ensure high effectiveness in the surgical management of parastomal hernias.
