Multimodality TTE and cardiac CT diagnose a sinoatrial nodal artery-to-right atrium fistula in an infant.
Why the study?
Coronary cameral fistula is a rare congenital anomaly characterized by an abnormal communication between a coronary artery and a cardiac chamber, where multimodality imaging is needed to establish the anatomical diagnosis and interpret hemodynamic significance alongside coexisting shunts.
Case Report (n=1)
Multimodality imaging with transthoracic echocardiography and cardiac CT accurately defines rare coronary cameral fistulas and their hemodynamic significance alongside coexisting left-to-right shunts to guide conservative management in infants.

Supports conservative follow-up for restrictive infant fistulas with shunts; leaves open generalizability pending larger cohorts.
Coronary cameral fistula is a rare congenital anomaly characterized by an abnormal communication between a coronary artery and a cardiac chamber. We report a 7-month-old infant with a restrictive sinoatrial nodal artery-to-right atrium fistula identified on transthoracic echocardiography and further defined by cardiac computed tomography. Additional lesions included an ostium secundum atrial septal defect and a small patent ductus arteriosus, together contributing to the overall left-to-right shunt burden. Baseline electrocardiography showed sinus rhythm without sinus node dysfunction or clinically significant atrial arrhythmia. The case highlights the value of multimodality imaging in establishing the anatomical diagnosis, interpreting hemodynamic significance in the presence of coexisting shunts, and supporting conservative follow-up in a rare coronary anomaly.

Lone et al. (2026) conducted a case report in Restrictive sinoatrial nodal artery-to-right atrium coronary cameral fistula with ostium secundum atrial septal defect and small patent ductus arteriosus (n=1). Multimodality imaging (transthoracic echocardiography and cardiac computed tomography) and conservative follow-up was evaluated on Anatomical diagnosis and hemodynamic interpretation. Multimodality imaging with transthoracic echocardiography and cardiac CT established the diagnosis of a sinoatrial nodal artery-to-right atrium fistula in a 7-month-old infant.
