This case report describes the first known instance of an aortic mycetoma caused by the dematiaceous mold Exserohilum following ascending aortic aneurysm repair and aortic valve replacement.
We present the first reported case of aortic mycetoma due to Exserohilum, a dematiaceous mold. The patient presented with neurologic symptoms 2 months after root-sparing ascending aortic aneurysm repair with aortic valve replacement. Multimodal imaging revealed irregular densities in the aortic lumen and graft, consistent with infectious thrombus. Redo aortic aneurysm repair and aortic valve replacement was performed, and a culture of the extracted thrombus confirmed the diagnosis.

Zhuang et al. (2026) conducted a case report in Aortic mycetoma due to Exserohilum (n=1). Redo aortic aneurysm repair and aortic valve replacement was evaluated on Diagnosis and surgical extraction of infectious aortic thrombus. Multimodal imaging and surgical re-intervention identified a 2.3 × 3 cm aortic graft mycetoma caused by Exserohilum rostratum resulting in luminal narrowing down to 24 × 15 mm.
