Key result
Home BP-guided lifestyle counseling lowers systolic BP by ~10 mmHg vs. standard counseling in workplace hypertension.
Why the study?
Workplace health checkups may identify elevated BP, but a single measurement may not reflect usual BP, prompting evaluation of home BP-guided lifestyle management after elevated BP detection.
Does smartphone-connected home blood pressure monitoring combined with home blood pressure-guided physician-led lifestyle counseling reduce systolic blood pressure at the subsequent annual workplace health checkup compared with standard lifestyle counseling without home blood pressure feedback in municipal employees with elevated workplace blood pressure?
RCT (n=58)
Open-label
Stratified randomization according to sex, age, and systolic blood pressure

Does smartphone-connected home blood pressure monitoring combined with home blood pressure-guided physician-led lifestyle counseling reduce systolic blood pressure at the subsequent annual workplace health checkup compared with standard lifestyle counseling without home blood pressure feedback in municipal employees with elevated workplace blood pressure?
Incorporating smartphone-connected home blood pressure monitoring to guide physician-led lifestyle counseling significantly reduced systolic blood pressure and HbA1c at 1 year compared with counseling without home blood pressure feedback in employees with elevated workplace blood pressure.

Background Workplace health checkups may identify elevated blood pressure (BP), but a single measurement may not reflect usual BP. We evaluated home BP‐guided lifestyle management after elevated BP was detected at a workplace health checkup. Methods Fifty‐eight municipal employees were randomized to home BP monitoring plus physician‐led lifestyle counseling ( n = 31) or the same counseling without home BP feedback ( n = 27). Both groups received two scheduled counseling sessions. The primary outcome was change in systolic BP at the subsequent annual health checkup. Results Compared with the control group, the intervention group had greater reductions in systolic BP (between‐group difference, −9.9 mmHg; 95% CI, −17.0 to −2.8; p = 0.007). After excluding three participants who initiated antihypertensive medication, the systolic BP difference remained significant (−7.7 mmHg; 95% CI, −14.6 to −0.8; p = 0.029). Conclusions Home BP‐guided lifestyle counseling may improve management of elevated BP detected at workplace health checkups.

Omama et al. (2026) conducted an RCT in Elevated blood pressure detected at a workplace health checkup (n=58). Smartphone-connected home BP monitoring plus physician-led lifestyle counseling vs. Physician-led lifestyle counseling without study-provided home BP feedback was evaluated on Change in systolic blood pressure (SBP) at the subsequent annual workplace health checkup (MD -9.9 mmHg, 95% CI -17.0 to -2.8, p=0.007). Home BP-guided lifestyle counseling significantly reduced systolic BP compared with standard counseling (MD -9.9 mmHg; 95% CI -17.0 to -2.8; p=0.007) in employees with elevated workplace BP.
