Congestive Heart Failure (CHF) represents one of the most frequent chronic diseases associated with poor quality of life, reduced social activity and patient mobility, especially in old patients. During CHF management, both the patient and caregivers should hold an active role in the type of necessary care need. Considering that the majority of patients (or caregivers) are not completely aware of the necessary care plan, direct medical supervision is crucial. According to the recent national guidelines on tele-homecare, an in-depth reorganization of health care is necessary to identify needs from the hospital setting to homecare management in urban area, which in turn will play an important role on improving personal independency levels and quality of life.
