Introduction: Prevalence of OSA has been found to be significantly higher in recent studies from Western countries, but still there is dearth of epidemiological studies involving level I PSG from India. Aims & Objectives: 1. Approximate the prevalence-estimates of OSA 2. To determine the diagnostic accuracy and performance validation of preselected screening tools in reference to level I PSG. Methodology: For this facility based cross sectional study, healthy volunteers (18-80 years) were randomly selected and underwent level I diagnostic sleep study at sleep lab. Diagnostic accuracy of various sleep questionnaires was assessed with respect to OSA (AHI≥15). A novel questionnaire was devised and internally and externally validated in 450 individuals. Results: Out of 727 participants (51% males) Age 40. 0 years (31. 0, 48. 0) BMI: 23. 3 Kg/m2 (20. 6, 26. 6): moderate-severe OSA (AHI≥15) was seen in 236 individuals (32. 5%). Factors associated with OSA were Age, BMI, Neck circumference, HTN, Waist Circumference, and Diabetes Mellitus. AHI was divided into quartiles: 4th Quartile was significantly associated with hypertension, Diabetes mellitus, Triglyceride and Metabolic syndrome (pConclusion: In one of the largest studies ever done with level I PSG, OSA was found to be highly prevalent (32. 5%). This prevalence was much higher than seen in previous studies. New screening tool has better accuracy than conventional screening tools for predicting OSA in Indian settings.
