In this modern robotic era, Robot Assisted Simple Prostatectomy (RASP) is wide spreading, and Hugo RAS by Medtronic is one of the main robotic platforms in the market. According to the configuration proposed by Medtronic, the four arms should be used simultaneously, with the bed assistant placed on the patient right side. In our experience, a simple three arms configuration, showed satisfactory outcomes. In the present video, we aim to show a step-by-step RASP according to Millin's technique, with an easy-friendly three arms configuration, useful to avoid clashing during procedure. METHODS: Between June and September 2023, two patients underwent RASP in two tertiary referral centers through Hugo RAS. The first 11 mm robotic port was placed on the midline, targeting the prostate, just below the umbilicus. Other two 8 mm robotic ports were placed under vision, on the same transversal line, 3-5 cm below the endoscope, 10-12 cm from the optic. Two bed assistant laparoscopic ports were placed on the same line, 3-5 cm above the endoscope. Preoperative data, tilt and docking angles are reported as follows: (Figure 1). Retzius space reaching, prostatic capsule visualization and incision, traction stitches, transurethral catheter clamping and dragging, trigonization, and prostatic capsula suturing, were key-steps of the procedure and they were widely illustrated in the provided video. RESULTS: EBL were negligible in both cases, console and docking time were satisfactory. In both cases a benign prostatic hyperplasia was reported after Histopathological evaluation. After 1 month from surgery, no urinary symptoms were reported. The abdominal drainage was removed on the second postoperative day. The patients were discharged on the day after both. The transurethral catheter was removed after 1 week. CONCLUSIONS: Hugo RAS System provided excellent outcomes with a simple three-arms configurations in these Simpe Prostatectomy according to Millin's technique.Further procedures are needed to standardize the best docking and tilt-angles for RASP through Hugo RAS.
