Secondary infertility is the inability to conceive or carry a pregnancy to term after previously having a child, complexity of condition associated with previous recurrent miscarriages. Advanced maternal age, changes in reproductive health of both partners, environmental factors, systemic illness contribute to the heterogeneity of the condition. A 32-year-old female presented with a diagnosis of secondary infertility (BOH + left side hydrosalpinx + left ovary cystic lesion). She had also completed ATT treatment with a history of TORCH (IgG +VE). Other associated condition included pelvic inflammatory disease . Multiple factors were contributing to the disturbance of her reproductive system. The present case was treated with the Ayurvedic management protocol, started with Virechana for beeja suddhi, followed by Yoniprakshalan, Kshara Karma, Yonipichu dharana to manage PID, Basti-Yoga & Uttarbasti as per condition with medicated oil/ghrita for removing blockage of tube, better growth & maturity of follicles with release of ovum. The outcome of the treatment was that the patient conceived successfully and is currently in her third trimester. This case study signifies the approach of Ayurvedic principles in all disturbed factors to balance out, achieve a healthy reproductive system &conception without any consequences.
