Abstract Hilar cholangiocarcinoma, a common biliary tract malignancy, relies on surgical resection as the most effective treatment, yet postoperative prognosis remains suboptimal. Recent advancements in surgical management include preoperative biliary drainage, selection of hepatic resection approaches, innovations in bilioenteric anastomosis techniques, and the application of perioperative chemotherapy. This review synthesizes current literature on these developments, highlighting their clinical implications and unresolved challenges, and prospected the future research direction.
