Abstract Background Nutritional assessment and guidance are key elements in the management of inflammatory bowel disease (IBD). This study evaluated the effectiveness of a digital nutritional intervention (Nootric®) in improving nutritional and dietary parameters in patients with IBD in clinical remission. Methods A prospective study was conducted in patients with Crohn’s disease and ulcerative colitis followed at La Paz University Hospital. Clinical activity (Harvey-Bradshaw and Walmsley indices), biochemical remission (fecal calprotectin 150 µg/g; CRP 5 mg/dL), nutritional markers, and malnutrition risk (MUST) were assessed at baseline, 12, and 24 weeks. Patients used the Nootric® digital platform for personalized nutritional plans, interactive modules, and direct chat with a dietitian. Adherence to the Mediterranean diet (PREDIMED-14) and the global Nootric score (max = 58) were evaluated longitudinally Results A total of 151 patients were included; 110 completed 24-week follow-up (53% women, mean age 46.8 ± 12.8 years). Serum albumin and prealbumin levels improved significantly (4.38 → 4.49 g/dL, p = 0.003; 24.9 → 26.1 mg/dL, p = 0.047). No significant changes were observed in micronutrients, though baseline vitamin D deficiency (30 ng/mL) affected 58.1% of participants. The median PREDIMED score increased from 8.2 to 10.1 at 24 weeks (p = 0.021), indicating greater adherence to the Mediterranean diet. The Nootric score improved from 31 (IQR 26–36) to 40 (IQR 36–47) (p = 0.018). High engagement was observed, with 84.2% regular users and over 56,000 recipe interactions. Conclusion Digital nutritional support was feasible, well-accepted, and associated with improved nutritional markers and dietary adherence in IBD patients under stable disease control.
