Context: The anterior cruciate ligament (ACL) is the most commonly injured ligament of the knee joint, with various graft options available for its reconstruction. Aims: The aim of this study was to compare the functional outcome of ACL reconstruction using hamstring graft and peroneus longus graft. Settings and Design: Single-center retrospective comparative study. Subjects and Methods: International Knee Documentation Committee (IKDC), Tegner–Lysholm score of the hamstring graft and peroneus longus graft group compared at post-operative and on 1 year follow-up. Statistical Analysis Used: Independent sample t -test, paired sample t -test, Mann–Whitney U -test. Results: There is no statistically significant difference between the IKDC and Tegner Score on 1-year follow-up between peroneus longus and hamstring graft group. Conclusions: Peroneus longus can be considered an alternative and even as primary graft for ACL reconstruction with comparable results to the hamstring graft.
