**Abstract** Postpartum haemorrhage (PPH) is a leading cause of maternal mortality in sub-Saharan Africa, with resource-limited settings such as Northern Ghana's district hospitals being particularly vulnerable. This study evaluated the impact of a structured, simulation-based training programme on severe maternal outcomes. The intervention trained multidisciplinary maternity teams in the systematic 'HAEMOSTASIS' bundle for PPH management. A comparative analysis of hospital records was conducted for defined periods before and after the training. Maternal outcomes measured included the rate of severe PPH (blood loss ≥1000ml). Following the intervention, the proportion of cases progressing to severe PPH decreased from 8.2% to 5.7%. Qualitative feedback from staff indicated enhanced team coordination and protocol adherence. The findings suggest that simulation training was associated with a reduction in the incidence of severe PPH in this low-resource setting. It represents a promising strategy for strengthening emergency obstetric care. We recommend the integration of regular simulation training into continuous professional development programmes for maternity staff in similar contexts.
