Pharmaceutical policy is fundamental to effective health systems, but its formulation and implementation in East Africa present considerable difficulties. A regional analysis is required to understand the barriers to medicine access. This study aimed to critically analyse the development, implementation, and impact of national pharmaceutical policies in East Africa, identifying systemic barriers to equitable access to essential medicines. A qualitative, document-based critical review was conducted. The analysis examined regional policy documents, strategic plans, and peer-reviewed literature. Thematic analysis was used to identify key policy themes, implementation gaps, and stakeholder perspectives. Fragmented policy implementation was a dominant theme, resulting in inconsistent medicine availability. Supply chain weaknesses were identified as a primary cause of stock-outs in public health facilities, directly undermining policy objectives. Furthermore, policy formulation often lacked meaningful engagement with frontline healthcare workers. Pharmaceutical policies in East Africa are frequently compromised by systemic implementation failures, despite often being well-intentioned in design. This disconnect between policy intent and practical reality perpetuates inequities in access to essential medicines. Future policy processes must prioritise robust implementation frameworks with clear accountability. Strengthening regional supply chain mechanisms and institutionalising inclusive stakeholder consultation, particularly with end-users, are essential.
