Inequities in psychological health services continue to pose a major challenge to universal health coverage and overall population well-being. While mental health is increasingly acknowledged as a fundamental human right, access to effective and culturally appropriate psychological care remains highly uneven, particularly in low- and middle-income countries. These disparities are driven by a combination of weak health systems, adverse social determinants, persistent stigma, and insufficient policy prioritization. This narrative review adopted a structured, evidence-informed approach to examine global disparities in psychological health services and pathways toward more equitable care. A comprehensive literature search was conducted across five electronic databases, PubMed, Scopus, Web of Science, PsycINFO, and Google Scholar, and was supplemented by gray literature and policy documents from major global institutions. Publications from 2015 to 2025 were screened using predefined inclusion and exclusion criteria. In total, 38 eligible studies were included and analyzed through thematic synthesis. Analysis identified four interrelated themes shaping equity in psychological health services: (1) global patterns of inequity linked to social determinants and health system limitations; (2) community-based and culturally responsive models of care, including task-sharing, lay counseling, and peer support; (3) digital and technological innovations, such as telepsychology and e-mental health platforms, that help overcome geographic and stigma-related barriers; and (4) policy and health system reforms, including integration of mental health into primary care, universal health coverage, and multisectoral collaboration. Across settings, equity-oriented approaches were associated with improved access, acceptability, and sustainability when grounded in local contexts. Reducing inequities in psychological health services requires broad system-level transformation rather than isolated or short-term interventions. Sustainable progress depends on coordinated policy reform, meaningful stigma reduction, culturally grounded service delivery, and multisectoral collaboration that addresses the social determinants of mental health. By synthesizing diverse strands of evidence within an equity-focused framework, this review offers practical insights to support the development of inclusive, resilient, and people-centered mental health systems worldwide.
