ABSTRACT Background and Objective: Hypospadias management varies globally despite established surgical techniques. We compared practices between Sociedad Iberoamericana de Urologia Pediatrica (SIUP) and Hypospadias International Society (HIS) members to identify regional variations and inform evidence-based guidelines. Study Design: This is a cross-sectional survey of pediatric urology specialists from SIUP and HIS societies conducted between April and June 2025. Results: A total of 127 specialists participated (83 SIUP, 44 HIS; response rate 42.3%). HIS members demonstrated significantly higher surgical volumes (18.2% vs 8.4% performing >10 cases monthly, P = 0.031) and greater technique modification frequency (88.6% vs 60.2%, P = 0.001). SIUP members preferred earlier intervention (<12 months: 41.0% vs 27.3%, P = 0.045) and tubularized incised plate urethroplasty for distal cases (68.7% vs 52.3%, P = 0.018). HIS members more frequently utilized the Mathieu technique (34.1% vs 15.7%). Both groups demonstrated limited systematic quality-of-life assessment (<20% using validated instruments). Conclusions: Significant practice variations exist between international pediatric urology societies in hypospadias management, particularly regarding the surgical volume, timing preferences, and technique selection. These findings emphasize the urgent need for evidence-based consensus guidelines and standardized long-term outcome assessment protocols to optimize care quality worldwide.
