Introduction and Objective: Diabetic ketoacidosis (DKA) is a serious complication of diabetes. This study examined differences in morbidity, mortality and costs of DKA-related hospitalizations in people with type 2 diabetes (T2D), stratified by primary or secondary DKA diagnosis. Methods: We searched the 2022 National Inpatient Sample database for individuals with T2D, using ICD-10 codes (E11.xx, any position). Hospitalizations were stratified by: (1) DKA (E11.1, E11.10, E11.11) as the primary diagnosis at discharge, or (2) DKA documented in secondary positions. We estimated incidence of DKA (per 1,000 T2D hospital stays), and compared patient characteristics, length of stay (LOS), mortality, and costs. Results: Among 7,612,098 hospitalizations in people with T2D, 1.3% (n: 99,735) had DKA as the primary reason for admission (13.1/1,000), and 1.2% (n: 91,935) had DKA as a secondary diagnosis (12.6/1,000). Compared to DKA as the primary diagnosis, people with a secondary diagnosis of DKA were older, hospitalizations had longer LOS, higher mortality rates, and higher mean costs (Table). Conclusion: People with T2D hospitalized for other reasons with secondary diagnosis of DKA had significantly higher mortality, longer LOS and higher cost. Studies limited to hospitalizations with a primary DKA diagnoses may underestimate the true impact of DKA on mortality and cost of this condition. Disclosure R.J.
